Purpose:

The Preventative Needs Assessment Survey will help us understand the issues that could cause problems for your child, so we can work on preventing them. Similar to how doctors have found that eating too much fatty food can cause heart issues, researchers at the University of Washington have discovered reasons that could lead to risky behaviors. By using the Preventative Needs Assessment Survey to gather more information about these reasons, community-based organizations can develop programs to reduce the likelihood of substance use issues in your child's community.
